Acceptance and Admission

ADMISSION ASSESSMENT |
|
Once you have chosen Generations at Lowry to be your new home, an on-site assessment will be completed with you
to ensure that you meet the admission criteria and to determine your
required level of care. Our criteria for admission is available upon
request. |

THE ADMISSION PROCESS |
|
After the assessment is completed, the next
step is to have your primary care physician complete our physician's
assessment form. Because this form must be completed prior to admission,
we are happy to assist you in communicating with your physician as needed. Once the physician's assessment is complete, we will
schedule a move-in date and provide you with our move-in packet to help
you get acquainted with your new home. On or before the day of admission,
we ask that you plan time to go over the admission agreement with our
team. |

The criteria for admission and continued stay
at Generations at Lowry will be determined by your current needs
for services. Every resident of Generations at Lowry is continually
assessed to ensure that we are meeting and can continue to meet their
needs. |

SERVICES AND LEVELS OF CARE
|
|
INDEPENDENT LIVING |
|
A resident who fits into the Independent Living category
is one who is functionally and socially independent. In addition to the
apartment, monthly rent includes three meals per day, recreational
activities within and outside the facility, weekly housekeeping,
transportation for health and non-emergent medical needs, linen service,
and environmental security. The resident is responsible for all of
their other needs such as bathing, medications, etc. Personal laundry
services and a meal discount program are available upon request. |

ASSISTED LIVING |
|
Residents who fit into the Assisted Living
category are functionally and/or socially impaired and require 24 hour
protective oversight and/or assistance with personal care. Generations at
Lowry offers three levels of Assisted Living services. Prior to
moving in, our team will complete an assessment with you in order to
determine which level of care is needed. While you may be admitted at
one level, the team will continually reevaluate your needs to
ensure that you receive the care you require. Therefore, you may
move in and out of the different levels of care. While the following descriptions
provide a general guideline to the three classifications, our aim is to
consider each individual's unique situation in evaluating their Assisted
Living level of care. |

prompting/cueing, minimal assistance with dressing, safety checks,
oxygen monitoring, transports or escorts to meals and activities, daily
trash removal, assistance with grooming, daily bed making, and weekly
personal laundry services. |
